Position Summary
Case Manage 10 VA families and 10 LAHSA families referred to
Westwood Transitional Village (WTV) and provide supervision and
support to each family while residing here. Case Manager will also
conduct outreach, provide information and referral services,
supportive counseling, classes, and group facilitation, provide
when needed, transportation and other services as requested.
Essential Functions
Working Conditions
Ability to walk, stand, bend squat climb, kneel, and twist on an
intermittent or sometimes continuous basis. Ability to be able to
lift up to 50lbs. Ability to grasp, push, pull objects such as file
cabinet drawers and reach overhead. Ability to operate computer,
fax, and telephone.

Qualifications
EducationBachelors of Social Work (required)
ExperienceAt least two years' experience working with at
risk/homeless families. (required)Bachelor's degree in human
services or related field or equivalent services. (required)
Licenses & CertificationsDriver's License (required)
